Criteria:
-Complete and submit the application
-Must be a senior, or a junior graduating early, from a Tooele County School District high school. Students graduating from adult education are also eligible to apply.
-Minimum cumulative GPA of 3.0 or higher throughout high school (certified transcript required)
-Essay, not to exceed 800 words. The essay should include information about yourself, your understanding and knowledge about organ donation, and why you are planning a career in the nursing/medical field.
-Leadership and community service
-List of medical-related classes and/or experience
Award:  $500 (6 awards)
